Community Health Team Dundee
We provide a wide range of support and our team is experienced in community-led approaches in various topic areas, including: Mental health and wellbeing Substance use Keeping active Food, nutrition and healthy eating.
SOS (Sources of Support)
The SOS service is for patients aged 16 or over whose mental health and wellbeing are affected by social and economic issues such as debt, housing, benefit support or similar. The link workers negotiate, liaise, advocate and communicate on the patient's behalf at the same time as they help to build the capacity and resilience of the patient to deal with issues themselves If you're interested in accessing the service please contact reception.
Stroll and Roll Session
There are so many benefits to walking. Feel free to join the Stroll and Roll sessions at Douglas Community Centre Join our Stroll and Roll session Mondays 11:30am at Douglas Community Centre Dundee followed by an optional hot drink and snack at Doug Support Cafe.

First Contact Team
The First Contact Team provides a first point of contact for anyone living in the community, aged 16 and over, and who does not have either a District Nurse or an Active Social Work worker. First Contact provides a quick referral, quick assessment and where possible- a quick service. Customers are benefiting from this faster service which is improving referrals and assessment times for people in the city.
First Contact provides a service for customers, their relatives and carers.
